You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
If you wish, Dr. Wong can provide a pre-visit assessment of your smile using “Smile Selfies” as well as any x-rays you may have. All you have to do is take four pictures. There is an option to attach files and documents after pressing the Complete and Send button when sending your Patient Registration Form. We will be able to determine if we can “fix your smile” with either braces or Invisalign. We will also let you know if orthodontic treatment is covered by your insurance, and if treatment can wait or should be started right away. Of course, the final decision on treatment goals, strategies and cost will be determined at your personal visit with us.
